Creating a dynamic form in javascript with a dropdown

I am currently trying to create a dynamic form using Javascript. I am trying to create a form using an array of objects. Reading from the array and appending them into a form and then create a dropdown with the numbers from 0 to the max. I haven't attempted the dropdown bit yet as I want to first get the form to appear on the screen.
