Facebook connect process on my web site

I create Facebook connect for my web site but it is nothing happend i just get white screen and in console there is no errors.

For that facebook connect i download this from github and in that folder i put my code. Here is my index.php i don't know why is this happend.

I also create facebook app and get my ID's like that :
