Source code of Registration form in Angular.js is not working

i an newbie to angular.js . i am developing a registration form in angular.js. I found a source code for Registration form http://ift.tt/1CHzbmT .

I used this source code from view page source option. I generated different folders for angular.js and bootstrat.css which are library files in that code.. But while running that code the error messages are also coming along with the form. I am not getting what to change in that code. can anyone please help me.
