how to use html5 app cache manifest to save index.html?

I have a single page web app that I compile and minify all my js css and html into a single index.html file so for my app cache manifst file I would have thought I just needed to have this in my app manifest file:

app.appcache: CACHE MANIFEST /index.html

but when I hit the page in chrome it logs: Creating Application Cache with manifest http://ift.tt/1BDG3Tp gorillaseed.net/:1 Application Cache Checking event gorillaseed.net/:1 Application Cache Downloading event gorillaseed.net/:1 Application Cache Progress event (0 of 1) http://ift.tt/1AA9fFO gorillaseed.net/:1 Application Cache Error event: Resource fetch failed (4) http://ift.tt/1AA9fFO

So it appears to be working in terms of making an effort to fetch /index.html but it fails, which seems a little crazy since it is running from /index.html in the first place. How do I get it to store my index.html?
